Personal Information Collection Statement
Southern District Health Centre (SDHC) is committed to take reasonable and practicable steps to protect the privacy, confidentiality and security of any personal information collected from a client in the DHC and CDCC Scheme, in accordance with the requirements of the Personal Data (Privacy) Ordinance (PD(P)O) (“The Ordinance”) and the guidance provided in the information leaflets and other publications issued by the Office of the Privacy Commissioner for Personal Data as well as any other relevant requirements under the laws of Hong Kong.
Purpose of Personal Data Collection
SDHC collects appropriate amount of, but not excessive, personal data by fair and lawful means. Provision of your personal data is on a voluntary basis. The data so collected are for any lawful services and events of SDHC and AKA. Failure to provide accurate/complete information may affect SDHC’s ability to process your application or provide healthcare services. Please ascertain your data are accurate and complete and notify SDHC if there are any changes.
Use of Personal Data
Personal data provided will be used by SDHC for one or more of the following purposes:
1. Improvement of efficiency, quality, continuity and integration of healthcare provided to the clients of SDHC.
2. Processing membership applications.
3. Verification: SDHC staff will collect personal data when any person registers online or in person. The data will be used to identify the person concerned and to contact them if necessary.
4. Feedback: SDHC collects any feedback for research purposes and to help optimize our services and events in future.
5. For direct marketing purposes according to your indication. (Please refer to the “Use of Personal Data in Direct Marketing”.
6. Such purposes of use as permitted by or under the laws of Hong Kong e.g. court proceedings and criminal investigations.
Data Accuracy
SDHC will take reasonable and practicable steps to ensure the accuracy of personal data.
Data Security
SDHC will take appropriate steps to protect the personal data against any loss, unauthorized access, use, modification or disclosure.
Data Sharing with Third Parties
Personal data will only be accessible to SDHC staff and will be disclosed to the following third parties only when necessary:
- Personnel or organizations to which you have agreed to disclose data; and
- Related parties that are authorized by law, or with which SDHC is required to share data.
SDHC will not sell, rent or submit personal data to other organizations.Access and Correction of Personal Data
You have the right to request access to, or correction of, personal data, except in cases specified by the Ordinance. Your access right includes a duplicate copy of the document after you submit your request and pay for the fee. Use of Personal Data for Direct Marketing Purposes
SDHC will use your personal data (including name, mobile number, fax, email and mailing address) as appropriate to update you on the work of SDHC, which may cover member and volunteer recruitment, fundraising, courses, events, etc. in a e-news format via direct messaging (e.g. WhatsApp, SMS, WeChat), mail, email and telephone. SDHC will only use your data for such purposes with your consent. You have the right to request SDHC to stop using your personal data for direct marketing purposes (see below Opting out of Direct Marketing).
Opting out of Direct Marketing
SDHC respects your right to opt out of direct marketing. You can request SDHC to stop using your personal data at any time by notifying us in writing. SDHC will then remove you from our mailing list.
